Before you start answering the LPI questionnaire, it is important that you read the instructions below carefully.

Completing this questionnaire will produce a confidential, detailed, feedback report that your LPS consultant will share with you.

The questionnaire contains quantitative questions (you have 4 answer choices on each question - Almost Never, Not Often, Quite Often, Almost Always) and 4 qualitative questions at the end where you will be asked to write comments about yourself (or the person you are evaluating if you are a 360 degree rater.)

In order for the feedback report to be useful, it is essential that you are open and honest with your answers - remember the only person you are fooling is yourself if you do not answer honestly.

The total questionnaire should take around 20 minutes maximum and you should answer each question with an immediate, instinctive response. In other words, try not to reflect too much on the questions - just go with your gut feeling.

Also, you must answer all the questions.

Try to make sure you are relatively relaxed when doing the questionnaire and complete it in a quiet, undisturbed space (that means turning off your mobile phone!).
